HST is the combination of the Provincial Sales Tax (P.S.T) and Government Sales Tax (GST). For Ontario, it is the combination of 7% and 6% to make the HST rate for Ontario 13%.
Depending on which province you live in, the sales tax rate may vary. Some provinces only charge GST and some have a higher tax rate. This information is irrelevant when it comes to the resident of the vendor as the applicable tax will be dictated by the province in which the goods are sold.
You do not need to register separately for GST and HST. They are combined as one CRA account when you register. It is up to the taxpayer who is providing the service to determine the correct tax rate based on where the sale has occurred.
It is every business obligation to collect the correct amount of GST/HST when providing a service. If the wrong amount is collected than the taxpayer could be subject to paying the difference back along with penalties and interest.
When invoicing customers, it should state which tax is being charged, the tax rate and you should also provide the customer with your GST/HST number.
